Older adults who suffer from a variety of health conditions often need healthcare in different settings to meet their many needs. This transitional type of care is defined as a set of actions designed to ensure the coordination and continuity of healthcare as patients transition between different locations or different levels of care. From Landplan Engineering's experience, these locations for the different levels of care are identified as Independent Living, Assisted Living and Nursing Care. Each location requires our design team to focus on differing aspects of the residents/patients' needs. Independent living projects are designed for flexibility and freedom. Assisted living sites require a higher interest in mobility issues yet need the atmosphere of freedom. Finally, nursing care requires attention to the caregiver's ability to assist the residents and for ample space to be used for passive activity.
